Our terms and conditions are designed to ensure a positive experience for both patients and the practice. Here are some key points:

• Appointment Policy: If you are an NHS patient and you fail to attend your appointment without giving us a minimum of 24 hours’ notice you will be sent a warning letter. If you fail to attend a second time you will be sent a follow up letter to inform you that, in line with NHS guidelines and practice policy we will no longer be able to offer you treatment and you will need to find an alternative dentist.

  If you are a private patient you will be asked to pay a deposit when you book an appointment. If you fail to attend without notifying us at least 24 hours in advance, the dentist reserves the right to retain these monies. You will then be required to pay another deposit when you rebook the appointment.

• Financial Responsibility: Patients are responsible for the payment of all services provided.

• Privacy: We are committed to protecting your privacy and confidential information. Please refer to our Privacy Policy for more details.

Dental practices are only responsible for a patient whilst they are undergoing an active course of treatment. A dental practice does not have to accept a patient for a further course of treatment if they have reached the capacity of their NHS contract and patients are equally free to move between dental practices without the need to register with them.

Yes you can! We have recently introduced this feature. You will now be sent the appropriate forms for completion with the text confirming your appointment. This saves you time when you arrive at the practice – and we’re doing our bit for the planet too!

If you are a NHS patient you must notify us if your circumstances change and you become exempt or if you no longer qualify for free treatment because you are no longer receiving benefits.

We require you to bring evidence of any exemption so that this can be added to your file.

If you do not inform the receptionist of a change of circumstances and make a false claim for free NHS dental treatment you will receive a fine from NHS Business Services. This will add £100.00 to the cost of your dental treatment and surcharges are added for late payment of the fine.

We do not have a car park at Forest Dental Care. However, we can arrange Ringo parking for you at a charge of £3.00. You can park in WSC or HS zones – please notify the receptionist which zone you are in when you arrive at the practice. Please allow time to park and arrange for your permit prior to your appointment.
